La pand\u00e9mie de COVID-19 a amplifi\u00e9 cette dynamique, soulignant la n\u00e9cessit\u00e9 de solutions plus int\u00e9gr\u00e9es et coordonn\u00e9es pour g\u00e9rer les pics d&rsquo;afflux et de d\u00e9bordement de patients. <\/p>\n\n<p class=\"wp-block-paragraph\">Dans ce contexte, nous avons propos\u00e9 une strat\u00e9gie centralis\u00e9e de r\u00e9affectation des patients, ax\u00e9e sur les cas non urgents. L&rsquo;id\u00e9e est d&rsquo;exploiter les d\u00e9s\u00e9quilibres entre la demande et la capacit\u00e9 des \u00e9tablissements : lorsqu&rsquo;un h\u00f4pital est satur\u00e9, une partie de la charge peut \u00eatre redirig\u00e9e vers des \u00e9tablissements moins encombr\u00e9s, ce qui am\u00e9liore la r\u00e9partition des patients dans le syst\u00e8me et r\u00e9duit l&rsquo;encombrement global. <\/p>\n\n<p class=\"wp-block-paragraph\">La mise en \u0153uvre a \u00e9t\u00e9 d\u00e9velopp\u00e9e dans un r\u00e9seau de 4 salles d&rsquo;urgence affili\u00e9es \u00e0 un seul ASST avec un nombre annuel d&rsquo;acc\u00e8s sup\u00e9rieur \u00e0 100 000.<\/p>\n\n<h2 class=\"wp-block-heading\">Strat\u00e9gie m\u00e9thodologique<\/h2>\n\n<p class=\"wp-block-paragraph\"><strong>\u00c9tape 1 &#8211;<\/strong> Mod\u00e8le d&rsquo;<strong>optimisation stochastique<\/strong><br\/>D\u00e9veloppement d&rsquo;un mod\u00e8le stochastique pour optimiser la r\u00e9affectation des patients non urgents dans des sc\u00e9narios d&rsquo;incertitude, avec les objectifs suivants :<\/p>\n\n<ul class=\"wp-block-list\">\n<li>minimiser le temps total dans le syst\u00e8me (attente + traitement)<\/li>\n\n\n\n<li>\u00c9quilibrer la demande et la capacit\u00e9 entre les h\u00f4pitaux\/cliniques<\/li>\n<\/ul>\n\n<p class=\"wp-block-paragraph\"><strong>\u00c9tape 2 &#8211; Mise en commun des patients et crit\u00e8res d&rsquo;\u00e9quit\u00e9<\/strong><br\/>Introduction de strat\u00e9gies de mise en commun des patients et de contraintes d&rsquo;\u00e9quit\u00e9 pour garantir la r\u00e9affectation :<\/p>\n\n<ul class=\"wp-block-list\">\n<li>flexible (adaptable aux pics)<\/li>\n\n\n\n<li>\u00e9quitable (\u00e9viter de faire attendre les patients au-del\u00e0 d&rsquo;intervalles raisonnables),<\/li>\n<\/ul>\n\n<h2 class=\"wp-block-heading\">R\u00e9sultats<\/h2>\n\n<figure class=\"wp-block-image size-large\"><img fetchpriority=\"high\" decoding=\"async\" width=\"1024\" height=\"661\" src=\"https:\/\/rigenerai.com\/wp-content\/uploads\/2026\/03\/riallocazione-data-driven-della-domanda-in-network-di-strutture_LR-1024x661.jpg\" alt=\"\" class=\"wp-image-2430\" srcset=\"https:\/\/rigenerai.com\/wp-content\/uploads\/2026\/03\/riallocazione-data-driven-della-domanda-in-network-di-strutture_LR-1024x661.jpg 1024w, https:\/\/rigenerai.com\/wp-content\/uploads\/2026\/03\/riallocazione-data-driven-della-domanda-in-network-di-strutture_LR-300x194.jpg 300w, https:\/\/rigenerai.com\/wp-content\/uploads\/2026\/03\/riallocazione-data-driven-della-domanda-in-network-di-strutture_LR-768x496.jpg 768w, https:\/\/rigenerai.com\/wp-content\/uploads\/2026\/03\/riallocazione-data-driven-della-domanda-in-network-di-strutture_LR.jpg 1123w\" sizes=\"(max-width: 1024px) 100vw, 1024px\" \/><\/figure>\n\n<p class=\"wp-block-paragraph\"><strong>Meilleure gestion des flux<\/strong><br\/>L&rsquo;approche \u00e0 la demande <strong>r\u00e9duit les temps d&rsquo;attente de 25 \u00e0 30 %<\/strong>, acc\u00e9l\u00e9rant l&rsquo;acc\u00e8s aux soins et augmentant le d\u00e9bit.<\/p>\n\n<p class=\"wp-block-paragraph\"><strong>Optimisation de la \u00ab\u00a0flotte\u00a0\u00bb\/transferts<\/strong><br\/>La strat\u00e9gie de pooling augmente le nombre de patients r\u00e9affect\u00e9s en moyenne, <strong>r\u00e9duisant les co\u00fbts de la flotte de 5 \u00e0 10 %<\/strong>.<\/p>\n\n<p class=\"wp-block-paragraph\"><strong>Plus de stabilit\u00e9 dans la planification des ressources<\/strong><br\/>La strat\u00e9gie de routage coh\u00e9rente <strong>r\u00e9duit les temps d&rsquo;attente d&rsquo;environ 10 %<\/strong>, ce qui rend l&rsquo;affectation des ressources plus pr\u00e9visible et facilite le dimensionnement optimal de la flotte.<\/p>\n","protected":false},"excerpt":{"rendered":"<p>Dans les syst\u00e8mes de soins de sant\u00e9 o\u00f9 la demande est variable et les ressources limit\u00e9es, l&rsquo;encombrement des salles d&rsquo;urgence peut cro\u00eetre rapidement, augmentant les temps d&rsquo;attente et les inefficacit\u00e9s op\u00e9rationnelles.
