Cependant, la planification des \u00e9quipes est compliqu\u00e9e par trois facteurs r\u00e9currents : la n\u00e9cessit\u00e9 d&rsquo;une couverture 24 heures sur 24 et 7 jours sur 7, la p\u00e9nurie de personnel et la pression constante sur la ma\u00eetrise des co\u00fbts. Dans les mod\u00e8les traditionnels, souvent bas\u00e9s sur une planification manuelle ou des r\u00e8gles statiques, des inefficacit\u00e9s typiques se produisent : surcouverture dans certaines plages horaires (co\u00fbts et heures suppl\u00e9mentaires inutiles), sous-couverture dans d&rsquo;autres (risque clinique-op\u00e9rationnel et d\u00e9t\u00e9rioration de l&rsquo;exp\u00e9rience du patient), stress et \u00e9puisement du personnel, ce qui entra\u00eene un roulement et de l&rsquo;absent\u00e9isme. Pour r\u00e9pondre \u00e0 la demande variable de soins (quotidienne et hebdomadaire), il faut une approche dynamique et fond\u00e9e sur des donn\u00e9es, capable d&rsquo;adapter syst\u00e9matiquement les niveaux de couverture aux besoins r\u00e9els.<\/p>\n\n<p class=\"wp-block-paragraph\">Nous avons mis en place un syst\u00e8me d&rsquo;optimisation qui permet de d\u00e9finir les niveaux de personnel et de g\u00e9n\u00e9rer des \u00e9quipes individuelles, dans le but d&rsquo;am\u00e9liorer la couverture, la durabilit\u00e9 et les co\u00fbts d&rsquo;exploitation.<\/p>\n\n<p class=\"wp-block-paragraph\">Le mod\u00e8le est con\u00e7u pour :<\/p>\n\n<ul class=\"wp-block-list\">\n<li>aligner la couverture infirmi\u00e8re sur la demande pr\u00e9vue (et sa variabilit\u00e9)<\/li>\n\n\n\n<li>r\u00e9duire les heures suppl\u00e9mentaires et l&rsquo;inefficacit\u00e9<\/li>\n\n\n\n<li>am\u00e9liorer l&rsquo;\u00e9quilibre et la qualit\u00e9 des affectations de personnel<\/li>\n<\/ul>\n\n<p class=\"wp-block-paragraph\">La mise en \u0153uvre a \u00e9t\u00e9 d\u00e9velopp\u00e9e dans un h\u00f4pital de taille moyenne, complexe et multidisciplinaire, avec des contraintes cliniques et organisationnelles h\u00e9t\u00e9rog\u00e8nes et la n\u00e9cessit\u00e9 d&rsquo;une couverture continue.<\/p>\n\n<h2 class=\"wp-block-heading\">Approche m\u00e9thodologique<\/h2>\n\n<p class=\"wp-block-paragraph\"><strong>\u00c9tape 1 &#8211; D\u00e9finir les niveaux de dotation (optimisation robuste<\/strong>)<br\/>Un mod\u00e8le d&rsquo;optimisation \u00ab\u00a0robuste\u00a0\u00bb d\u00e9termine les niveaux de dotation optimaux en tenant compte de l&rsquo;incertitude de la demande des patients, en \u00e9vitant les solutions fragiles qui ne fonctionnent que \u00ab\u00a0dans des conditions id\u00e9ales\u00a0\u00bb.<\/p>\n\n<p class=\"wp-block-paragraph\"><strong>\u00c9tape 2 &#8211; G\u00e9n\u00e9ration d&rsquo;affectations individuelles (programmation mixte en nombres entiers)<\/strong><br\/>Un mod\u00e8le de programmation math\u00e9matique construit les affectations individuelles en respectant les contraintes op\u00e9rationnelles et organisationnelles, notamment :<\/p>\n\n<ul class=\"wp-block-list\">\n<li>les \u00e9quipes et les cr\u00e9neaux horaires<\/li>\n\n\n\n<li>dotations pour le personnel en formation<\/li>\n\n\n\n<li>bin\u00f4mes tuteur\/soutien<\/li>\n<\/ul>\n\n<h2 class=\"wp-block-heading\">R\u00e9sultats<\/h2>\n\n<figure class=\"wp-block-image size-full\"><img fetchpriority=\"high\" decoding=\"async\" width=\"737\" height=\"477\" src=\"https:\/\/rigenerai.com\/wp-content\/uploads\/2026\/03\/assegnazione-turni-infermieristici.png\" alt=\"\" class=\"wp-image-2315\" srcset=\"https:\/\/rigenerai.com\/wp-content\/uploads\/2026\/03\/assegnazione-turni-infermieristici.png 737w, https:\/\/rigenerai.com\/wp-content\/uploads\/2026\/03\/assegnazione-turni-infermieristici-300x194.png 300w\" sizes=\"(max-width: 737px) 100vw, 737px\" \/><\/figure>\n\n<p class=\"wp-block-paragraph\">L&rsquo;introduction du syst\u00e8me a produit des avantages mesurables sur la qualit\u00e9 du service, l&rsquo;exp\u00e9rience du personnel et les co\u00fbts :<\/p>\n\n<ul class=\"wp-block-list\">\n<li><strong>Am\u00e9lioration de la continuit\u00e9 de la<\/strong> couverture des <strong>soins<\/strong>: couverture plus coh\u00e9rente et mieux adapt\u00e9e aux fluctuations de la demande, avec moins de probl\u00e8mes li\u00e9s \u00e0 une sous-organisation.<\/li>\n\n\n\n<li><strong>Am\u00e9lioration de la satisfaction du personnel<\/strong>: horaires plus \u00e9quilibr\u00e9s et plus pr\u00e9visibles, r\u00e9duction des d\u00e9s\u00e9quilibres et des risques d&rsquo;\u00e9puisement professionnel.<\/li>\n\n\n\n<li><strong>R\u00e9duction des co\u00fbts d&rsquo;exploitation<\/strong>: R\u00e9duction des co\u00fbts globaux gr\u00e2ce \u00e0 une utilisation plus efficace des ressources et \u00e0 un recours moindre aux heures suppl\u00e9mentaires.<\/li>\n<\/ul>\n\n<p class=\"wp-block-paragraph\">Une <strong>r\u00e9duction des co\u00fbts totaux de 6 \u00e0 8 %<\/strong> a \u00e9t\u00e9 observ\u00e9e lors d&rsquo;une mise en \u0153uvre, ce qui a permis d&rsquo;\u00e9viter la sous-couverture.<\/p>\n","protected":false},"excerpt":{"rendered":"<p>La planification des \u00e9quipes de soins infirmiers est de plus en plus complexe : la couverture 24 heures sur 24 et 7 jours sur 7, la p\u00e9nurie de personnel et la demande variable de soins rendent inefficaces les mod\u00e8les traditionnels fond\u00e9s sur des r\u00e8gles statiques ou une planification manuelle.
